Flu Vaccine Clinic for Ravenwood Students

Ravenwood is fortunate to offer a Flu Vaccination Clinic for all students who turn in a completed consent form. Flu vaccine provided by the Anchorage School District is only in the inject-able (shot) form. No Flu Mist (nasal spray) will be available for administration through the district.
Flu Injection is an inactivated form of the virus and is administered by injection into the upper arm. It takes up to 2 weeks following vaccination for the body to develop protection.
If you are interested in getting your child vaccinated, you will find a Consent Form and the Vaccine Information Sheet below, in electronic copies attached to an email you should have received, and at the school website https://www.asdk12.org/Ravenwood. Forms can also be picked up in the front office.
Please return all completed consent forms to Nurse Cottle or the front office, and please feel free to contact Nurse Cottle @ 742-3254 with any questions. Forms will not be accepted the day of the clinic. NO CONSENT = NO VACCINATION
